- Add --web mode to Orchestrator, ArmController, Arm, PatientMonitor
- Embedded HTTP servers (cpp-httplib) serving per-device web UIs
- Fix ArmControllerWebApp self-deadlock on PAUSE/START/SHUTDOWN commands
- Fix Patient Monitor waveform to freeze when vitals data goes stale
- Add shutdown overlay + tab-close handling to web UIs
- Add kill_demo.sh helper script

Add the --vscode launcher mode, a local VS Code webview extension with a two-by-two app layout and Ctrl+C tab cleanup, launcher tests, and updated Digital OR documentation. Include web UI shutdown handling improvements.
