feat(cloudflare): diagnose failed checks, and react when they fail - #22

…art on its own when they fail Pull request sessions get github_pull_request_checks, listing the checks on the latest commit with their IDs, and github_check_failure, showing a failed check's output, annotations, failed steps, and the end of its job log, with the whole log saved in the workspace. Saving long tool output moved to ToolOutput.ts, shared with bash. When checks fail, time out, need action, or cannot start on the latest commit of a pull request the app follows, onSubscribedPrEvents sends the AgentSession a request listing them and hands the delivery off. The agent fixes and pushes if the cause is clear, and otherwise explains the cause and what to change. Older commits' checks and passing checks are ignored.
