Remove Codecov and all test coverage tooling: the CI upload and README badge, pytest coverage options, pytest-cov and its unused dependencies, coverage exclusion comments, and report ignore rules.
Validation: all 200 tests, ty, pre-commit hooks, and portable lockfile checks pass. No coverage references remain in tracked files.
