fix: 사이트용 신청 폼 PUT 엔드포인트 삭제 - #435

네비게이터가 PUT /api/studies/{studyId}/application-form 으로 신청 폼을
덮어쓸 수 있는 문제를 수정한다.
specs/authz-guards/spec.md B절 후속(2026-10-04 기획 확정)에 따라
사용자 사이트 PUT 을 삭제한다. 신청 폼 저장은 백오피스
PUT /api/admin/studies/{studyId}/application-form (@RequireAdmin, 캡틴만) 단일 경로로 통일.
