Consolidating #466-#471 here (same reporter, same device, same v1.39.0). We see 6 instances within ~2 hours -- this indicates the failure-loop detector (force_steady_state) is working, but the underlying gene gene_gep_repair_from_errors cannot handle the error shape in your sessions.

To move this forward, we need:

1. Upgrade -- v1.39.0 is far behind. Latest is v1.69.12+ which has reworked repair-gene behavior and a new fallback innovation path when the repair gene is banned:

   npm i -g @evomap/evolver@latest

2. One full session jsonl from memory/sessions/ showing the actual error the repair gene is attempting to fix (redact sensitive paths/tokens).
3. evolver --version after upgrade.

What v1.39 -> v1.69 changed that is relevant here:

• New force_innovation_after_repair_loop trigger now actually chains into gene_innovate_capability instead of re-arming repair.
• Failure ratio threshold recalibrated so consecutive_failure_streak_10 is reached less often on transient errors.
• Session log excerpt sanitization is better (redacts tokens we can see leaking in your excerpt).
